Routine hospice care is a level of end-of-life care that provides comprehensive support to patients and their families in their homes or other residential settings.

General Inpatient (GIP) Hospice Care provides symptom management in a facility setting when symptoms are too severe to be controlled at home, typically involving intensive pain or symptom control, frequent medical evaluation, and 24-hour nursing care.

 Medical management in the home for up to 24 hours a day when medically appropriate. 

 Temporary care provided for up to 5 consecutive days in nursing home,  hospice, inpatient facility, or hospital so that the family member or  friend who is the patient's caregiver can take some time off. 

Hospice is a comprehensive plan of care and support that focuses on the comfort and quality of life for terminally ill patients to live as comfortably as possible in the final stages of their lives. Angel's Grace provides hospice care with compassion and thoughtfulness using pain relief and symptom management strategies.

 Eligibility is determined through a physician's certification stating  the patient has a terminal diagnosis and a prognosis of approximately  six (6) months or less if the disease were to run its normal course.

Hospice services are covered from a variety of sources including:

• Medicare
• Medicaid
• Private Insurance Companies
• Donations & other Private Payments

Patients and/or their families should verify the scope of their individual insurance coverage.

Palliative Care is an interdisciplinary medical care giving approach aimed at optimizing quality of life and mitigating suffering among people with serious, complex and often terminal illnesses.

 Any patients with a serious, long-lasting disease or life-threatening  illness are eligible for palliative care at any stage of their illness.
